۱) Blood composition — ترکیب خون و پلاسما

حجم خون بزرگسال حدود ۵ لیتر است. کل خون از دو بخش اصلی تشکیل می‌شود: plasma حدود ۵۵٪ و formed elements حدود ۴۵٪. پلاسما تقریباً ۹۰٪ آب دارد و بخش باقی‌مانده شامل مواد غذایی، الکترولیت‌ها، گازها، آلبومین، عوامل انعقادی، آنتی‌بادی‌ها، مواد دفعی، آنزیم‌ها و هورمون‌هاست. pH پلاسما در متن حدود 7.4 ذکر شده است.

نکتهٔ آموزشی: برای سؤال‌های عددی، «۵۵٪ پلاسما / ۴۵٪ formed elements» را با «۹۰٪ آب در پلاسما» قاطی نکن.

۲) Blood cells — RBC, WBC & Platelets

تمام سلول‌های خونی در red bone marrow ساخته می‌شوند. RBCها فراوان‌ترین عنصر سلولی‌اند و اکسیژن را به کمک hemoglobin حمل می‌کنند. WBCها در دفاع و ایمنی نقش دارند و platelets در hemostasis و شروع انعقاد شرکت می‌کنند.

نکتهٔ آموزشی: اندازه و تعداد نسبی سلول‌ها را ببین: RBCها بسیار فراوان‌اند، WBC بزرگ‌تر و هسته‌دار است، platelet بسیار کوچک‌تر است.

نکتهٔ آموزشی: band cell را با monocyte اشتباه نکن؛ band یک neutrophil نابالغ است.

Exam traps

  • Neutrophil شایع‌ترین WBC است.
  • Monocyte بزرگ‌ترین WBC معرفی شده است.
  • Lymphocyte کوچک‌تر از monocyte و در adaptive immunity بسیار مهم است.
  • Band cell = immature neutrophil، نه immature monocyte.

۴) Hemostasis & coagulation — هموستاز و انعقاد

hemostasisتوقف خونریزیhem/o = blood · -stasis = stoppingبا thrombosis یکی نیست.

پس از آسیب رگ، platelets به محل آسیب می‌چسبند و platelet plug می‌سازند. سپس عوامل انعقادی فعال می‌شوند و در واکنش نهایی fibrinogen → fibrin تبدیل می‌شود. رشته‌های fibrin سلول‌ها و پلاسما را گیر می‌اندازند و clot می‌سازند.

نکتهٔ آموزشی: سؤال‌های واژگان اغلب precursor و product را جابه‌جا می‌کنند: fibrinogen precursor است، fibrin محصول نهایی رشته‌ای.

۵) Blood typing — ABO, Rh & cross-matching

نکتهٔ آموزشی: در شرایط اورژانسی، متن O Rh-negative را به‌عنوان خون RBC با کمترین احتمال القای پاسخ مطرح می‌کند؛ اما cross-match کامل‌تر از ABO/Rh است.

۶) Immunity — innate vs adaptive

۷) Sources of adaptive immunity — active & passive

نکتهٔ آموزشی: Hematocrit درصد حجمی RBCهاست؛ dehydration می‌تواند درصد RBC را به‌صورت نسبی بالا ببرد، بدون اینکه تولید واقعی RBC افزایش یافته باشد.

نکتهٔ آموزشی: در متن، sickling با آزادکردن اکسیژن از Hb و رسوب مولکول غیرطبیعی مرتبط است؛ انسداد رگ‌های کوچک و hemolysis ایجاد می‌شود.

Reticulocyte count در افتراق علت anemia مهم است: افزایش آن نشان‌دهندهٔ افزایش تولید RBC در پاسخ به از دست‌رفتن یا hemolysis است؛ کاهش آن به نفع اختلال تولید مثل nutritional deficiency یا aplastic anemia است.
